Mutual Funds
Step 1: Complete the new account application with your investment professional
The new account application can be downloaded, printed, and completed by hand or you can fill in the application online and print it out. After filling out the application, each registered owner must sign it and your investment professional's firm must complete the dealer section

MFS cannot establish your account unless Sections 1 through 6 of the application are completed. (Download application below.) Tips for setting up your account
- Make sure to register the names on the account the same way you register other financial accounts. For example, if you sign your name and have other accounts registered "John R. Jones," make sure to use the same format on your MFS account

- Fill in the application completely and print clearly. Leaving out information may delay getting your money invested or we may have to send the application back.
- Link your checking/savings account information so you can quickly and easily add money to or withdraw money from your mutual fund account.
- Individuals: Consider completing a "Transfer on Death" (TOD) registration request to quickly transfer proceeds to your named beneficiaries in the case of death.
- Trusts, corporations, partnerships, limited liability companies (LLC), and other organizations:
-Provide complete documentation including, as applicable; the Articles of Organization/Incorporation, Trust Agreement, or partnership agreement.
-Provide documents identifying person(s) with authority to over the account.
